Individuals with valid PPO dental insurance or who are fee-for-service (out-of-pocket) patients.

This promotion is not available to patients with HMO, Medicaid, or other non-PPO plans.

2. Promotional Offer Details
The promotion entitles eligible patients to receive one (1) complimentary in-home teeth whitening kit.
The whitening kit will be provided after completion of the new patient exam, x-rays, and professional dental cleaning, regardless of the presence of other dental conditions such as cavities or gum disease.
Use of the whitening kit before completing all diagnosed dental treatment is strictly discouraged.
3. Patient Responsibility
